新日鐵住金㈱大分製鐵所２高炉熱風炉GL+10m床にて、塗装補修用の足場を仮設する作業中、500Aの配管上を移動中墜落し死亡しています。
この災害では、安全帯を取るための親綱は設置されていましたが、被災者は安全帯を使用していませんでした。

重大災害を受けての安全衛生部長の指示事項は、
① 作業前に安全保護具の装着状況をチェックリストを用いて確認してください。
② 作業中の安全保護具の使用状況を現場にて確認してください。

また、高所作業では、安全柵の設置、安全帯の使用、開口部の養生、安全ネットの設置等々、二重三重の安全対策を実施してください。
